Baby Dedication Request:

The pre-requisites for submitting a Baby Dedication form are as follows:

• The parent(s) of the baby must be a member in good standing at Grace Church of All Nations.
• The parent(s) of the baby have been attending church regularly for the past six months.

If you have met the above pre-requisite requirements, please complete and submit the Baby Dedication Request form below. You must also drop off a Xerox copy of your baby’s birth certificate at the Church Office as soon as possible. Note, a Church Administrator will contact you to finalize the date of your Baby Dedication.

** Baby Dedications are done on the last Sunday of each month.
